1) Complete the DS-160 by following the step on this video
According to travel.state.gov the DS-160 is an online « Nonimmigrant Visa Application form, for temporary travel to the United States . The DS-160 is submitted electronically to the Department of State website via the Internet. »
2) Apply at the US ambassy of your choice
Of course outside of the US. Because the goal of the visa is to allow you to « entry » in the US. Because you cannot reentry in the place you are already. That’s why you cannot renew your us visa while you are in the us.
