A 65-year-old man with a dual-chamber pacemaker placed for sick sinus syndrome (SSS) had 2 strokes attributed to inadvertent left ventricular pacemaker lead placement. In this video, a subcostal four-chamber view from a transthoracic echocardiogram (TTE) zoomed in to the interatrial septum, left to right flow (red-orange jet) through an atrial septal defect (ASD) demonstrates how the lead entered the left ventricle (LV) and became a focus for clot with embolism.
